Girls Forms
All girls must have the following updated documents on file with the troop before being dropped off at any AHG meeting or activity. All forms are now combined in one convenient document. DO NOT email these forms. A paper copy for each girl should be printed, filled out, and turned in.

Adult Volunteer Forms
All registered adults must have an updated health & medical form on file with the troop before they can attend any AHG meeting or activity as a registered adult.

[Download not found] – this form is required when girls or adults will be participating in any activity American Heritage Girls considered to be “High Adventure”. This form MUST be completed by a licensed medical professional.
[Download not found] – AHG requires all adults who wish to volunteer for most* positions within the troop to apply for membership, consent to a background check, and complete several training courses, including a child-safety training course. Non-registered adults are not allowed to participate in overnight troop activities.
*Event planning volunteers are not required to be registered unless the event is an overnight activity.
